Epoprostenol is a platelet aggregator inhibitor also known under the brand name of Flolan and Veletri, used to manage hypertension in patients with heart failure. It is synthesized from prostaglandin endoperoxides in human vascular tissue. Once administered intravenously, it travels directly through the bloodstream acting on platelet receptors P2Y purinoreceptor 12, causing adenylate cyclase to produce cAMP further inhibiting platelet activation. It prevents coagulation as cAMP acts on a calcium transporter decreasing intracellular calcium levels that are needed for thromboxane A2 binding. It is metabolized to 2 metabolites and eliminated via the urine.
