Icosapent is an important polyunsaturated fatty acid (eicosapentaenoic acid (EPA)). It is mostly found in fish oils. A diet rich in this molecule can be used/started to lower elevated triglycerides level in patients who are hyperglyceridimic. Icosapent may also help as a therapeutic agent for patients with cystic fibrosis by reducing disease severity and it may play a similar role in type 2 diabetic patients. In general, icosapent lowers serum lipid concentration as well as the incidence of cardiovascular disorders and prevents platelet aggregation (blood clot). This molecule also inhibits the arachidonic acid conversion into the thromboxane-2 and prostaglandin-2 families because it inhibits the prostaglandin G/H synthase 1 and 2 (COX-1 and COX-2). The anti-inflammatory, antithrombotic, and immunomodulatory actions of EPA are probably due to the role of the molecule in eicosanoid synthesis. EPA is metabolized into leukotriene B5 (LTB5) and thromboxane A3 (TXA3), which act as eicosanoids that promote vasodilation, inhibits platelet aggregation, and suppress leukocyte chemotaxis. Then icosapent shows anti-atherogenic and anti-thrombotic effects in cells. EPA serves as a substrate for Prostaglandin-endoperoxide synthase 1 and 2. Furthermore, it influences the function and binds to the Carbohydrate responsive element binding protein (ChREBP) and a fatty acid receptor (G-coupled receptor) called GP40. This pathway shows the mechanism of action of icosapent on the prostaglandin G/H synthase as an analgesic. This drug is mostly found as an oral capsule or tablet.
